1. Agreement to Terms
These Terms of Service (“Terms”) constitute a legally binding agreement between you (“you,” “user,” or “patient”) and Revano Life, LLC ("Revano Life", “Revano,” “we,” “us,” or “our”) governing your access to and use of our website, platform, and related services (collectively, the “Services”).
By accessing or using the Services, you agree to these Terms. If you do not agree, you may not use the Services.
2. Nature of the Services
Revano operates an online health education and care navigation platform that helps users connect with independent, licensed healthcare providers and third-party service providers.
Revano is not a medical provider and does not:
Practice medicine
Provide medical advice, diagnosis, or treatment
Prescribe medications
Dispense or fulfill prescriptions
All medical services are provided solely by independent licensed healthcare professionals and their affiliated organizations.
3. Telehealth Services (Integrated Notice)
3.1 Use of Telehealth
Through the Services, you may access healthcare provided via telehealth technologies, including video visits, audio communication, secure messaging, and digital intake forms.
Telehealth services are delivered by independent licensed healthcare providers, not by Revano.
3.2 Benefits and Limitations
You acknowledge that telehealth may provide improved access and convenience but also has limitations, including:
No in-person physical examination
Reliance on information you provide
Potential need for in-person follow-up care
Possible technical disruptions
Not all medical conditions are appropriate for telehealth evaluation.
3.3 Emergency Disclaimer
Telehealth services are not intended for medical emergencies.
If you are experiencing an emergency, call 911 or seek immediate in-person care.
3.4 Provider Independence
All medical care is provided solely by independent licensed providers who are responsible for:
Clinical evaluation and diagnosis
Treatment decisions
Prescribing medications (if appropriate)
Revano does not interfere with or control clinical judgment.
Clinical decisions, including diagnosis, treatment, and prescribing, are made solely by independent licensed providers without influence from Revano.
3.5 No Guarantee of Care or Prescription
You acknowledge that:
A provider may decline to treat you
A prescription may not be issued
Treatment decisions are solely at the provider’s discretion
Revano is not responsible for the acts, omissions, or clinical decisions of independent healthcare providers.

Services are available in all 50 U.S. states through affiliated medical providers and pharmacy partners operating in accordance with applicable state licensure requirements. The Pharmacy Hub states that it is licensed by state boards of pharmacy nationwide and supports prescription fulfillment in all 50 states.
Please note that certain medications, formulations, or fulfillment options may vary depending on state-specific pharmacy regulations, prescribing requirements, and dispensing restrictions

4. Eligibility
You must be at least 18 years old and legally capable of entering into contracts to use the Services.

You agree to provide accurate information and maintain the security of your account credentials. You are responsible for all activity under your account.
6. Scheduling and Provider Access
Revano may facilitate referrals, scheduling, and intake submissions to healthcare providers. We do not guarantee:
Provider availability
Acceptance of your case
Timing of appointments
Treatment outcomes
7. Payments and Insurance
You may be charged for services provided by third-party healthcare providers or pharmacies.
Depending on the provider:
Insurance may or may not be accepted
Cash-pay models may apply
Payment may be processed via third-party processors
Revano does not set clinical pricing.
8. Prescriptions and Pharmacy Fulfillment
If a provider issues a prescription:
It is issued solely by the provider
It may be sent to a third-party pharmacy
Fulfillment, pricing, and shipping are handled by the pharmacy
Revano does not dispense medications or operate a pharmacy.
9. User Conduct
You agree not to:
Misrepresent your identity or medical information
Use the Services for unlawful purposes
Attempt fraudulent access to prescriptions or care
Interfere with platform operations
10. Intellectual Property
All content, branding, and platform materials are owned by Revano Life, LLC or its licensors and are protected by intellectual property laws.
11. Disclaimers
THE SERVICES ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS.
REVANO DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT.
We do not guarantee:
Medical outcomes
Provider availability
Platform uptime or error-free performance
12. Limitation of Liability
To the maximum extent permitted by law, Revano Life, LLC shall not be liable for any indirect, incidental, consequential, or punitive damages.
Total liability shall not exceed amounts paid to Revano (if any) in the preceding 12 months.
13. Indemnification
You agree to indemnify and hold harmless Revano Life, LLC from claims arising out of:
Your use of the Services
Your violation of these Terms
Misuse of healthcare services
14. Termination
We may suspend or terminate access to the Services at any time for violation of these Terms or for legal/compliance reasons.
15. Governing Law
These Terms are governed by the laws of the State of Delaware.
16. Changes to Terms
We may update these Terms from time to time. Continued use of the Services constitutes acceptance of the updated Terms.
